Choice Health Network Upper Cumberland office moves to new location

Choice Health Network’s Upper Cumberland office in Cookeville, Tennessee, has moved to a new location that is wholly dedicated to the organization’s mission.

The office, previously located inside the Upper Regional Health Department, will continue to serve the area’s most vulnerable individuals and families affected by HIV, mental illness, substance use, homelessness and LBGTQ+ discrimination.

“Expanding to serve more people across Tennessee is the essence of Choice Health Network’s purpose,” said Steve Jenkins, CEO of Positively Living & Choice Health Network. “Opening our own office in the Upper Cumberland region is the next step to providing inclusive, compassionate healthcare throughout East Tennessee.

“I would like to thank all of the supporters and staff who have made this possible, as well as the Upper Cumberland Health Department staff who saw the need for our services and provided a space for us to launch here and grow. It’s a great moment for us as we strive to empower, promote and inspire wellness.”

The Choice Health Network Upper Cumberland office is now located at 225 N. Willow Ave., suite D, in Cookeville. Staff began seeing clients at the new location in late August.

The office currently provides housing support, including Housing Opportunities for Persons With AIDS (HOPWA); primary care and infectious disease care via telehealth; case management; mental health counseling; emergency food and transportation aid; and naloxone distribution.

Clients can use the site’s telemedicine suite for telehealth visits addressing a variety of needs, including mental health care, and then continue remotely or use the suite for subsequent visits if the client does not have access to adequate privacy, connectivity or technology.

“Offering an independent office helps our clients obtain services in a more discreet setting locally, instead of needing to drive to Nashville for the same types of services,” Jenkins said. “We want to remove any stigma and all barriers for those seeking care.”
